The team works together with nurse case managers to file pre-authorizations, maintain contact with your employer and the Workers’ Compensation office, and help you through the entire process.

Bracing & Medical Devices
The DME (Durable Medical Equipment) department at Commonwealth Orthopaedic Centers is committed to providing the highest quality products and services available to our patients. From crutches, canes and walkers to custom knee bracing, post-operative bracing and cold therapy, we provide a multitude of products at your doctor’s office to provide convenient “1-stop shopping” for your orthopaedic device needs.
Our skilled staff will take you through the entire process, explaining everything from application and maintenance to cost and insurance coverage. We strive daily to provide you with quality products at competitive pricing while working directly with your insurance company to secure payment and keep your out-of-pocket expenses as low as possible.
Please be aware that most DME products you receive from your physician will be billed to your insurance company and will be subject to your deductible and co-insurance per your plan. You will be held financially responsible for any product you choose to receive while at your appointment. It is your responsibility to verify your insurance coverage for any medical need. If you have specific questions, please ask to speak to a DME specialist at your next visit.

Your first copy is free. There is a $1.00 per page fee for additional copies.
We process these requests in the order in which they are received and attempt to have them completed within one week; however, please understand that this process can take up to 30 days.
Please note there is a $25 fee (per form) for forms that require completion by our office. Examples include: FMLA and disability forms.

To pay your bill, please complete our Online Bill Pay form or contact our Billing Department at 859.301.9508.
There is a $35 administrative fee for any returned refund checks due to undeliverable mail. There is also a $50 return check fee for any check not honored by your bank. Examples include: returned checks due to insufficient funds and accounts that have been closed.

Otherwise, please let us know when you arrive for your appointment, and we’ll be glad to get that ready for you during your appointment so that you have it when you leave.

Your child may need a “School Excuse” to take back to his/her school. If you know you will need a School Excuse at the time you schedule your appointment, please let us know at that time. Otherwise, please let us know when you arrive for your appointment, and we’ll be glad to get that ready for you during your appointment so that you have it when you leave.

If you need a prescription refill, please call us (859) 301-BONE (2663) or your pharmacy 48 hours in advance for any prescription refill, as it may take up to 48 hours for a physician to authorize the refill. Narcotic prescriptions will not be filled after hours or on weekends. Most prescriptions can be sent to the patient’s pharmacy electronically. Certain narcotic prescriptions cannot be sent to the pharmacy, and will require the patient or family member to pick up a paper prescription at the office. Prescriptions can be picked up at the front desk of each location.
The patient/family member picking up a paper prescription will be required to complete a Prescription Validation Form and provide a valid driver’s license/ID before receiving the prescription. Our office will keep a copy of the prescription validation form and prescription for three months. After three months all information will be shredded.
